IACUC Forms

Please complete and submit the appropriate form. Forms are submitted via Kuali Protocols.

Questions about Material Transfer Agreements can be directed to Technology Commercialization.

Visit to submit new protocols, amendments, and changes in personnel. Complete the relevant forms that are below and submit them via Kuali.  

All protocol notifications will come from Kuali Notifications, this includes expiration notices.  

  • New protocols: complete and upload the IACUC protocol to Kuali Protocols.
    • Complete the Word form, open Kuali protocols, click New Protocol, and then IACUC. Complete the baseline information, the personnel section, upload the protocol, and click submit.  
    • When responding to comments, please remember to highlight revisions, then navigator to the project in Kuali protocols, click replace in the upload section, and upload the revised protocol. 
  • Modification of existing protocol: complete and upload the modification form
    • Open Kuali Protocols, navigate to the project that needs to be amended, select amend in the right side column, and in the amendment section select "Procedural or Similar Change." Upload the modification request and click submit.  
  • Changes in personnel other than PI.  
    • Changes in PI must be made by filing an amendment. 
    • To add or remove personnel, navigate to the relevant project, select amend in the right side column, and in the amendment section select "Addition/Removal of Personnel." Navigate to the personnel section, click "+add line," complete the required information, and click submit.
    • Occupational Health, you must upload certificate of Form C.
    • CITI will autoload. If not, there may be a temporary code error, CITI may not have been completed via KSU SSO, or CITI is incomplete. If CITI will not autoload, you must manually merged and upload certificates.

  • FAQS:
    • Can a PI designate someone to submit on their behalf?
      •  A PI can assign someone else to manage the submission, but only the PI can click submit. Anyone else need to click, notify PI to submit. The PI will receive a notification to review and submit.
    • What's a validation error?
      • A validation error means a mandatory field has not been completed. A red exclamation point directs you to the location of the error. The majority of mandatory fields are in the personnel section. 
    • My project was approved via the "paper" process. Do I need to use Kuali?
      • Yes. All "paper" forms have been uploaded to Kuali. Follow the business processes outline above or contact researchcompliance@kent.edu for assistance. 
    • Why did we start to use Kuali?
      • A more sustainable business practice was needed. Additionally, Kuali will improve efficiency and transparency. 

 

Holding protocol - this can be used to provide care for animals that are not assigned to a protocol. This protocol only allows for typical husbandry and veterinary care. Form provided upon request.
